Sunflower Centre

 


The Sunflower Centre offers a range of services to patients affected by life-limiting illnesses, as well their families and carers. We are flexible and responsive to your needs, aim to enhance your independence and help you to live well. We do this by offering emotional, social, and spiritual support to manage your symptoms, and give you the time and space to relax.

What's on offer?

After your referral and intial assessment, we will agree a plan that best suits your needs. This may involve a range of services including: 

Relaxation & Re-enablement Day
Our nurse-led Relaxation and Re-enablement Day (R&R Day) is on Wednesdays from 10am until 3.30pm, providing social interaction, therapies, carer respite and peer support to help maintain independence and quality of life. Patients can attend for the whole day or part of the day for up to 10 weeks.

Chill Out Thursdays
Our 'Chill Out Thursdays' are held every week from 10am until 12noon and are open to all those living with or affected by life-limiting conditions. This is an opportunity to learn de-stressing and relaxation techniques, mindfulness, guided meditation and adapted yoga. All are welcome and refreshments are served.

Catch Your Breath (CYB) Workshop 
These workshops run every Thursday afternoon, and are for patients living with breathlessness and fatigue. The course covers restful sleep, nutrition and hydration, gentle and adapted exercise, and self-management techniques. Therapeutic relaxation and diversional techniques are also explored, from a non-pharmacological perspective. We offer 6 sessions to every patient, and family and carers may also attend.

Art Therapy
Every Thursday afternoon we run an art therapy workshop led by an art psychotherapist, for patients to explore emotions and issues in a safe and confidential environment. 

Drop-in
We run a Drop-in every Friday from 10am until 3pm, open to those living with life-limiting conditions, their families and carers. Pop in for information and social support, spiritual support, general advice, Advance Care Planning, art and creative work. There is no medical or nursing input at the Drop-in, so patients must be able to self-medicate. Refreshments are served and all are welcome - no referral or appointment is necessary, and family members and carers may attend independently of the patient.

Bathing Service 
We hold an assisted bathing service every Friday, with four one-hour appointments available. Many patients enjoy a complementary therapy and light refreshments before returning home. 

Lymphoedema Clinic
We hold a Lymphoedema Outpatient Clinic on Mondays and Thursdays, caring for cancer patients with mild to complex swelling.

Complementary Therapies
We have a range of complementary therapies, such as aromatherapy, Reiki, reflexology and Indian head massage are available for patients, their families and carers by appointment.

Volunteer Visitor Service
We have a volunteer visitor service offering regular visits from carefully matched, specially trained volunteers in patients’ own homes, for support, company, conversation and to help provide some carer respite.

Being referred as a patient

You can be referred to the Sunflower Centre by your GP, District Nurse, Community Specialist Palliative Care Nurse, Consultant or Clinical Nurse Specialist. We will always inform your GP and the other Healthcare Professionals involved in your care. Your initial assessment will determine your care plan which we will be reviewed with you during your time at the Sunflower Centre. Most services are offered on a time-limited basis.

Transport

A transport assessment will be completed for those attending the R&R Day, as we are unable to provide transport for all patients. Where possible we encourage family and friends to bring patients to the Sunflower Centre who need assistance.
