Your communication preference

We know that you care about St Leonard's Hospice and we'd like to stay in touch with you as we build our future - but will only do so if you tell us that's OK.

Please tick to confirm your consent and preference. If you DO NOT give consent we will not be able to contact you again about our work, fundraising activities and events.

If you're 16 or under, we can still keep you updated but need your parent/guardian's permission. We will post you a form for them to sign.

Declaration

Donations made are non-refundable. Due to space on the ribbon we are unable to add any personal messages. The ribbons will display names and significant date/s (if applicable) only.

We have suggested a minimum donation of five pounds. This is to ensure we cover the cost of the ribbon itself, staff time in writing ribbons or staffing 'write your own ribbon' events and the cost of sending your card to you. This means as much money can go towards patient care as possible.

Gift Aid

Please treat as Gift Aid donations all qualifying gifts of money made to St Leonard's Hospice. Today, in the past 4 years and in the future. I confirm I have paid or will pay an amount of Income Tax and/or Capital Gains Tax for each tax year (6 April to 5 April) that is at least equal to the amount of tax that all the charities or Community Amateur Sports Clubs (CASCs) that I donate to will reclaim on my gifts for that tax year. I understand that other taxes such as VAT and Council Tax do not qualify. I understand that St Leonard's Hospice will reclaim 25p of tax on every £1 that I give.
